Case Management
Case managers create a dynamic collaborative process which utilizes and builds on the strengths and resources of the consumer, family members, and community agencies to assist needs, access and coordinate needed services, and achieve short and long term goals. The case manager's process involves accessing, planning implementing, coordinating, monitoring and evaluating to promote quality cost effective outcomes. |
